If any of the below apply to you, please consider re-scheduling your appointment or having a Telehealth appointment 

1) Have you been in contact with anyone who has the COVID-19 virus?

2) Have you returned from overseas in the last 14 days, or have you been in contact with anyone who has?

3) Have you travelled interstate, or travelled by air, in the last 14 days, or have you been in contact with anyone who has?

4) Are you at all unwell with cold or flu symptoms, or fever?
